Questionnaire

Please note: We have created an abbreviated intake questionnaire to help us begin thinking about your potential case. Of course we will go into more detail with you directly, but this will help to start the dialogue. Also, we have found that by beginning the process it helps one to not feel so isolated and alone.

Please do not worry if you cannot answer all of the questions; just do the best you can.

1. Are you legally married, or if you are not allowed to legally marry, are you in a committed relationship? For how many years?

2. Do you already have any children? What are their ages? Are the children from the current marriage/relationship or a previous one, or both

3. Are you trying to have a child for the first time?

4. Do you think you will need any of the following: Donor Insemination Agreement, Ovum Donor Agreement, Surrogacy Agreement, Parenting Agreement, Nomination of Guardian, Authorization of Medical Treatment for Minor?

5. Is one of you going to bear the child and the other adopt (second parent adoption)?
Are you using a know or unknown donor?

6. Is one or both of you attempting to adopt?

7. Have you heard about the new legal procedure: “Petition to Determine Parental Rights” under the Uniform Parentage Act (UPA), (also known as the “intended parents”petition)?

8. If you are giving birth, do you have a power of attorney for health care already in place?

9. Do you already have a will? Does if provide for your child(ren)?

10. Have you heard of a “Living Trust?”

11. Are you concerned about family members not respecting your wishes?

12. Are there any special circumstances of which we should be aware?
